Is this on-going, or like 99% accurate?
It should be accurate. Much of it I have followed my self. I have already accounted for a few known errors in the guide. If there are errors someone will point them out soon enough.

Black Emporium items are the exception to the rule. Unlike the other stores it doesn't replace it's old stock when it gets the new stock each act.

I missed Fenris' gift in Act 2, and it's no longer there. Is this something I should worry about if I want him to stick with me (I'm on the mages' side)?

It shouldn't. It's easy enough to get his happiness max without it.

1. Cave on the east side of the bone pit
2. Dank Cave, an unmarked dungeon in the Wounded Coast, also were a lyrium source is located
3. Abandoned Thaig, This is accessed via the Recently Opened Passage entrance situated next to the western Sundermount exit
4. One in the Chantry
5.Viscounts Manor
Taking or reading any of these prevents you from being able to complete the quest. Reading a tome will give you +2 attributes but on the first time you do so, if you choose to take them you can sell them. Either reading or taking the tomes raises Merril's friendship Destroying them increases her rivalry
If you destroyed all 5. Then another will appear in Dark Town. Reading this one will give you the two attribute points while still completing the quest while destroying it will summon Xebenkeck who you need to defeat for a trophy

found on a Pile of Bones in a small ruin, south west of the Varterral Hunting Grounds Cave

I can give precise info on the elixir of heroism. I just ran into it now on my second playthrough. When you get into Hightown after the Qunari invasion, you'll fight a whole horde of carta thugs and qunari tribe. Right around the corner from that skirmish (along the path you need to take) in a corner of one of the turns, there is a corpse with the recipe on him.
Edit for clarity: It's due east from the Hightown Market "marker" on your map.

Also, the arcane tome in act 3 sold by korval's blades is 3 stat points, not one.

Black Emporium: Tome of Technique (+1 talent)
Black Emporium: Greater Tome of the Mortal Vessel (+2 Stats)
^Can't find these 2 items in the Black Emporium. Am I not seeing a hidden "shop" in there or something, cuz I've searched all over!

thanks for the clarification.
do you think you can add the gear that gives bonus to lock-picking?
for example, the dagger "Thrice Bound" that can be bought from the Lowtown weapon vendor but only in Act 1. I think there is a glove and possibly a helm and other items that give the bonus as well. They're missable, rare, and incredibly useful (makes it so you need less than 27 cunning to open master level chests); worthy enough to be included in your guide.

awesome list!
question about upgrades.
I know i got the portrait for bethany from the birthright quest, but dont recall picking up other. Do armor upgrades auto apply upon pickup? I am certain i picked up every thing shiny and glitter covered.
I have one filled of the five squares underneath her character in the armor screen in inventory. So i was wondering if it auto applied.
