The Stunts are planned by the wrestlers, and the storyline I believe is under the control of Hunter Hearst Helmsley. Most Injury's are real, unless one is combined in with the storyline. The wrestlers actually do feel pain though. I am not positive that most of that is true that's just what I have researched, and part of it is an opinion.

The storylines/feuds are conceived by a writing staff. The wrestlers more or less improvise the things they say on the microphone, but the things they say ultimately tie back to the dictated storylines. Several of the wrestling moves are faked. Punches, kicks, and piledrivers, for instance, are all faked. Some moves, like body chops or body slams, cannot be faked. Most of a wrestler's training has to do with not just learning wrestling techniques but with learning how to perform those techniques with as little risk of harm to the opponent as possible. Injuries are always a serious possibility during wrestling matches, and it is the wrestlers' duties to minimize the danger posed to the other people in the ring with them. Nonetheless, wrestling matches definitely hurt. As I said, some moves cannot be faked, and the wrestlers are definitely going to take some damage--just not as much as the home audience is made to believe.
"Professional wrestling" is all about creating an entertainment experience for the viewing audience. It's about spectacle and energy. Everything about professional wrestling is designed to get the audience excited.

- stunts are predetermined and so are storylines but you can't fake pain. They plan out the stunts to avoid major injuries that may put the careers and lives of wrestlers in jeopardy. Alot of notable wrestlers died during shows and I'm pretty sure those are real
