What are the differences between difficulty levels?

Particularly, what changes? enemy health? weapon damage? number of painkillers available? do you still get painkillers if you keep dying on all difficulty levels?

and what are the specifics of 'old school" and "hardcore" mode? I heard that there was no cover in one of these modes. That can't be possible, right?

Hard and Hardcore just mean you take more damage and there are less painkillers around. Old School means there is no cover system and no there is no final stand available when you go down with a spare painkiller. I'm not sure how the difficulty is compared to other modes, but I would guess the same difficulty as Hardcore.
