Scanner plug trophy was changed/removed. Don't know if you can update that though.

I've always done it on both parts. I believe it needs to be both. However I'm sure someone else can confirm this.
To get the trophies you need to pick up their "dolls", which can only be acquired by non-lethally killing their Beast forms. You will then need to pick up the doll during the Beauty fight that follows.

If I had previously unlocked a weapon through the password system, will I not get the trophy even if I do what is necessary to unlock it legitimately? For example, if I used a pssword to obtain the Patriot, if I got the BBE, would I be able to get the weapons trophy or will t be disabled?

I can confirm that the audio flashbacks on Shadow Moses are not required for the flashback trophy. I know there were 3 I missed because they simply wouldn't appear. However you may need to go through the full flashback cycle (you know how there's usually 3-4 pictures per flash back each time you press X you get a different one). The person that wrote the guide on PS3trophies.org probably missed the trophy because they didn't go through the complete cycle on one of them.

O man... BBE and Chicken would have been enough, maybe assassin too. O well. Already had 1 BBE save deleted, now my second is obsolete. 3rd times a charm I hope.

I hope somebody can confirm this because I only play during weekends. You can still load old saves, but you can't earn trophies through them, or you can not technically load them?

This is correct. You're still perfectly able to load up your old saves, but they can NEVER be used to to earn trophies. As your saves are time stamped, anything that has a date before the patched dropped will be disqualified from earning the trophies.
I have a question about the Sounds of the Battlefield (obtain all iPod tracks except those via password). Does the Subsistence song (obtained by playing a game of MGO) count towards this? If so, how else are we supposed to obtain the song if MGO is no longer available?

It can also be obtained by inputting mgo2play under Passwords, and therefore dos not count. Weapons and tracks that can be obtained by using passwords do not count towards the respective trophies.

I got through the first 4 acts fine but act 5 won't install for me
a disc with a question mark on it appears and Snake's animation freezes

That means your disc is dirty or scratched. It won't read it. You'll need to repurchase the game I guess...

I was just about to ask a question regarding the "Sounds of the Battlefield" trophy, but I found the answer myself XD. However, it might be interesting to let you guys know this, just in case.
Even if you start a new game from scratch, if you can already select from a number of songs on your iPod, it's because these are downloadable songs. However, all songs unlocked during the game must still be aquired once again (including the two songs from the code event in Act 4 which require two walkthroughts to aquire them both).
So don't panic if you think your carried-over user settings have unlocked some songs and ruin the achievement for you - because they don't.
