For the basic version, its what HBK_FAN_80 said but for a more in depth -
It probably easiest to go to ,(While in Universe Mode), Edit Superstars (Press X to select it), and then press X (Select) on the person you are trying to injure. You should be brought up on screen with the Show the superstar is on, Attributes, Finisher Moves, Crowd Reaction, and Abilities. Above "Crowd Reactions" there should be a section called " Hit Point Ratios " Select X to have a more in depth look. Once your on the Hit Point Ratio screen you may notice there are Percentages (%). The Limb with the smallest percentage has a better chance to get injured, while the Limb with the Highest Percentage has a better chance of not getting injured. (Example: Leg = 30% and Arm = 25%, this means that the Arm has a better chance of getting injured. So logically that is what you would be attacking/focusing on during the match). This is where Limb Targeting System (LTS) helps a lot in injuring someone. Using the example you should us LTS on the arm. ( Head= Triangle, Arm Left/Right= Square/Circle, Legs = X ). Then try getting the Arm in the Critical Zone (Red Zone). Even if the Arm is in the Critical/Red Zone, it isn't definite that the Superstar or Diva is going to get injured, it just raises the chance. I hope this helps!

There is no trophy for this, but I have injured someone, basically by getting them to critical damage in two or more body parts during a match.
